SLT2021 Semi-final - Greg v. Jim - Greg pitches P15 Keller 15 IP when he 
was allowed 12 IP.  Egregious 3 IP rule-break.  Not discovered until after 
Greg wins 4-2.  Greg and Jim replay Game 5 two hours later, with Greg winning 10-6. 

SL 2011, Keith clinches and pitches 5 free agent starters instead of
the max 2.  Jason Vargas, one of the 3 starters he kept from being
exposed to injury (and least effective, so would have been the pitcher
who Keith would have exposed the most in meaningless games), was not
allowed to pitch in the semis.  Keith also announced his rotation
ahead of time, though opponent Greg had already inferred what it would
be.

2009.06.15 - Matt's Annandale trades one of their starting pitchers.  Per 
the rules, they need to pick up a 21+ GS scrub to make one start since 
the acquired starter doesn't have enough rest.  Matt uses his 6th starter 
instead.  Matt volunteers to pick up the scrub for the next time his 6th 
starter would need to pitch.

2008.09.14 - Dave's Putumayo Salsa discovers Milton Bradley (of2.35 8.2rcg) 
has played two center field games too many.  Dave voluntarily agrees to play 
Josh Hamilton (of2.34 7.3rcg) two fewer CF games than originally allotted.

2001 or 2002 - Greg uses two too many SBA for Magglio Ordonez.  One of them was 
due to phone games.  Agreed upon solution is he can't use any SBAs in the 
playoff semifinals.
